Why does this happen

For some reason I get scared of being penetrated sometimes not just in my vagina but also my anus and mouth. I sometimes have a hard time having sex due to it. I try to relax but it won’t work, is it maybe stress? Same thing happens when I try to put tampons in.

Since my last comment wasn't proper enough 
I do think it's best to talk to someone who is unbiased.. a professional who might be able to assess your concerns and fears and work through them with you.
There are things that may or may not have happened that could have less you to the way you feel now.
If it's interfering with your life then I definitely think counseling, therapy, etc is a good starting point.
You can have phone conversations with therapists or psychologists to help you without having to face them and feel even more uncomfortable.
I just recently started therapy again and I honestly think a counselor has helped me a lot more than a psychologist I had for years.
I wish you the best and your fears are normal. You just might need some outside help to work on a solution to resolving or handling those fears better.
