Baby acne or rash?

My 4.5 week old baby has these bumps all over his face, mostly on his forehead and chin. Trying to figure out if it’s baby acne or a rash? ive been washing his face with mild baby soap and trying to keep it clean from spit up/ milk and putting baby lotion on 3x a day. We started introducing formula  2 weeks ago if that makes any difference ? thank you!

My last 3 babies had similar.. they had this around their little booty, neck, chin, and forehead.. They actually were little yeast-caused rashes according to my pediatrician who did a culture for me after my I had an all purpose ointment compounded for me at the hospital before i left and it took it away immediately. It was a mix of muprocin, nystatin, a low concentration steroid, and a couple other things.. I only used a tiny bit just on the areas w bumps, and stopped immediately when it cleared and followed w a standard diaper rash ointment and/or skin balm.  Id def talk to pediatrician, anf send pics:) good luck!! Adorable little baby!!♥️
