Help!!!

I'm 22 with twins. I have irregular periods I had one in January very light no cramps no boob soreness nothing. Went to dr not pregnant. Had spotting off and on whole month of February. Some in March with cramps. Bad back pain. Went to dr again not pregnant did urine test twice. I have a lot of symptoms that are not going away. If I'm not pregnant then what could it be??
