Nipple problem

I have what looks like a skin tag (for lack of better description) right on the tip of my nipple. It hurts really bad and I have put nipple cream on everyday and it is still really bothersome. This is my first pregnancy 34 weeks and I haven't started breast feeding obviously. I didn't know they would hurt this bad. Any one know what is going on and how to heal/reduce pain?
