Horribly painful urination; please help!

Just made a trip to the bathroom and when I went pee I had this AWFUL sharp pain when I went that I've never experienced before. I will note that I have kidney reflux so I've had more than my fair share of bladder infections and UTI's ranging from mild to extremely severe, and this is nothing like a burning/pressure pain associated with either of those. I also have interstitial cystitis, and even with both of my conditions this is something I've never felt/had before. It was so painful I could hardly force myself to continue peeing. I've been fine all day long up until now(it's now midnight) and I'm well hydrated. Any thoughts on what it could be?
