No choice for Drs and delivery hospital

Does anyone else not get a choice in which dr you get and where you have to deliver? With my first child I had to drive 50 miles away to a better hospital because that was the only way I didn't have to deliver at a hospital with a bad rep. This was because of the insurance. This time I can't do the same thing since I live in a remote area where there's not even anything 50 miles out. This is really frustrating.
