Baby peeing all over himself

My baby boy keeps peeing out of his diaper. It usually goes all over his back and head. I've tried multiple brands honest company, Huggies and pampers. He is two weeks and it is the most frustrating thing. I'm sure I am using the right size and putting it on right. Any ideas on what I can do?
