Matb1 form?

Question

When did you get you're matb1 forms? I checked online and I have to let my employer know when I am leaving by the 12th of march. I'm 22+2 but no midwives have mentioned a form. I don't have my next apt till 24 weeks.

Your midwife should give you one and it normally has to have the docs surgery stamp on it. 
Ask the midwife when you go, I think you have until 25 weeks to hand it in at work.

I got mine at the first midwife appointment after 20 week scan, they did forget though and I had to ask
