Meeting w/ Bishop

I've been needing to do this for a while but have just been scared... But I know I need to do it and think I am ready even though I'm still scared. I need to set up a meeting with my bishop to repent about a few things I've kept to myself for a while. But I have no idea how to do it. I haven't been to church for a while, just because I go with my sister and she's been sick for a long time and I didn't want to go without her. But do I just walk up to the bishop and tell him that I need to have a meeting with him? Or do I need to talk to someone else who sets up the meetings? How would I do that? I have no idea how or what to do :(
