Sex disaster

Has anyone had an injury due to sex? My husband and I were having sex and he pulled a little to far out and when he went to go back in (with all he had) he missed the hole and hit the side of me and I ended up bleeding and there was a tear on my vagina....so embarrassing lol but just wondering if this happened to anyone else.
