Employment history application help please!?

I'm confused  on top it says employer but on the bottom of that it says Name. So do i put my old jobs name where it says employer or my name? But what do i write where it says name?

I think you put your name. Probably the reason for that is because people might be known by that employer under a different name (like you worked there before you got married or something like that).

I think it should be the position instead of name, if the application is made by someone in the office it could just be a mistake. So it should be Employer: ex. Walmart Name(position): ex. Cashier. I wouldn't freak out as I've never seen this before and I've worked in HR its probably a mistake.

I'd probably just leave it blank..cuz it has an option for a supervisor name...maybe it's like...owner? Probably not....idk
