Confusing ultrasound picture

Help! I got this ultrasound done at 12 weeks. When the midwife took it she said my baby's feet were hiding. I clearly see the ribs and where I thought the head was (lower left). I showed it to my friends yesterday (one is a nurse and one has two kids) and they are convinced I am having twins because they think the extra white spot on the upper left is another head. What are your thoughts? Should I call and ask what that spot is? Now I'm freaking out haha Thanks!!
