Help hemorrhoids are going to be the death of me

I got hemorrhids after I delivered my daughter 7 years ago and now I am 35 weeks pregnant. I have about 5 the size of grapes. Two that are extremely painful feels like there is a nerve involved. I have done baths, preparation h, coconut oil, tucks pads, tea tree oil, witch hazel, tylenol.  Everything and I am no better. Any other suggestions I am going to call in the am to see if there is anything else my dr can do.
