if someone has died and they don't know the name of the person how would they contact their family

I'm not saying my boyfriend is dead but I haven't heard from him in awhile and I'm getting worried. I don't live near him and he lives by himself an stays in a shitty area. Let's say he was in the hospital would they have records of him would they have to turn his phone on to let them know or what? It's just I haven't heard from him and I hope nothing happened cause this has never happened before 😪
