hemorrhoids after birth

During my second pregnancy I had the worst case of hemorrhoid .. I didn't have em this last pregnancy but I'm 2 months PP now and have had em since birth but it goes away. Well a bit ago I went to the bathroom and it hurt really bad to poo , I look down and it's nothing but blood! Like a lot of blood.. I'm super worried now. Is there any over the counter remedies for this? Or do I have to go to my OB?
