miscarriage help

Last Friday I went to the ER for bleeding at 5 weeks pregnant. They did an ultrasound and saw an irregular gestational sac and yolk sac and sent me home with a threatened miscarriage. I saw my OB the following Monday and he did a vaginal ultrasound. He confirmed the irregularity of the gestational sac and also did not see a yolk sac. He told me I had a 99 percent chance of miscarrying and it should start within the next few days. He gave me the option for a d&c but said since it was still early to let my body try and do it naturally. He told me I should expect an hour or so of intense period cramps when the miscarriage happens. Since that Friday that I went to the ER I've continuously bled. Some times have been heavier then others.  I have passed a few good sized clots, but have had little to no discomfort or cramping. My breasts don't hurt anymore and my fatigue is gone. (Those were my only pregnancy symptoms). Emotionally I have lost hope that this can turn out well & I have come to terms with losing the baby. My question is, how do I know if I have already miscarried? Is it possible that I may have already miscarried and not have had severe cramps? Can my bleeding just be an after affect of the miscarriage? I'm super confused and just emotionally drained. I have a follow up appointment Tuesday with my OB and I will opt for the d&c if I haven't already miscarried. Just for my own mental clarity I'd like to try and wrap my head around what my bodies doing. Thank you for any insight you can provide ❤ I'm hopeful that we will have our rainbow baby in the future!
