22 year age gap??

My fiancé and I have a 22 year age gap. When we first went out, he didn't look old, but age didn't really matter in my situation. He healed scars previous men had made, and he opened up to me like I was the only one he could trust. We dated for 3 years until he proposed in Disneyworld, and now our wedding is less than a half a year away!! It will be a small wedding with close friends and family that support us. A beach wedding (very excited!!) He makes me feel like we'll be together forever. The thing is, most of our family doesn't approve, so that means we can't go visit them until they calm down about our relationship. My fear is that I know because of our age gap, he's going to pass first, if nothing happens to me. Also, there are tons of other problems we have to solve, but I don't know if I can truly do this. I love him, but I'm afraid of our future.
