NEED TO SEE THE GYNIE BUT NO INSURANCE??

So I have insurance, good insurance, but I don't have the insurance info. Like, legally, im covered under my biological father's insurance plan (I'm 17) but he's an abusive fuck face and I haven't seen him in 5 years and he won't give me the god damn insurance card. Does anyone know a way around this? I need my insurance info, I'm way past due for my STD test I get every six months, I need an exam, and I want to talk about switching my birth control. I've been to PP in the past, but they're still expensive if I don't have my insurance card. Anyone have any advice? Mom's divorced and a single parent and doesn't have much money. It was written into the divorce agreement that he would pay for my insurance, so we shouldn't have to pay OOP, even if we could afford it.
