If you've ever done pet shipping or know info help

I'm moving. I wanted to get my dog to be my emotional support dog on the plane to sit on my lap/free of charge. 👈🏼 All that info is directly from a airline so please don't go on how I'm wrong or i'm trying to scam the airline. I have a valid reason for wanting her to be my emotional support dog. The thing is the form that has to be filled out from a doctor. Which I understand! But I haven't been seen by a doctor in years and to get a note of any kind you have to be seen. I don't have insurance and limited $ to move anyways. So I'm not sure going about that is the right choice now.
