Who signs the birth certificate?

I'm 5 weeks pp and I realized I never filled out any paperwork after my son was born. I asked my husband about it and he said that he might have forged my signature for his birth certificate but he's not sure. I'm PISSED if he did since I was just right there the whole time!! Who signed your little ones birth certificate? Just one parent? Both? No one?

Both parents should sign. When I had my son the nurses were handing me so much paper work... I'm surprised they never gave you anything to sign!

I signed both of my kids' birth certificates. The nurses brought the paper work in 1-2 days after I gave birth (that way I knew exactly what was happening and what I was signing) 
My ex was no where in sight 💁🏻

I HAD to sign, the woman who brought the paperwork for it made me and said my husband could if he was around but didn't have to, i honestly dont remember if he did. But she watched me sign.
