You're not supposed to "pull out" before you ejaculate with a condom on. You're supposed to stop thrusting as soon as you start to ejaculate in order to A: prevent a blowout (which is how my sister got pregnant) and B: prevent tearing of the condom once its been ejaculated in. The man is then supposed to grip the base of the condom and slowly pull out AFTER he's finished ejaculating. The man would dispose of it... well, in whatevers the safest manner. Pulling out beforehand if anything can actually cause a snag and cause the condom to be sucked up into you WITH THE EJACULATE. That is why you should be taking birth control: unless you can 100% trust the man you're with to stop thrusting and use a proper condom from the very beginning (pre-ejaculate can get you pregnant and give you STDs) then you should be covering yourself from having a tragedy happen.
