Twins or no??? Ectopic???

Question

I am 4 weeks3 days and did an u/s at work. Thought maybe I saw two but also concerned about the white heart shape at the top around what I believe is the gestational sac and it's different than my other early scans so I worry about ectopic? Anyone else think maybe twins?? I've been way sicker this time than my last two starting right at 4 weeks but who knows! lol what's yalls opinions/comparisons to this pic?

Out of curiosity, what makes you think this could be an ectopic?  Ectopic pregnancies occur in the Fallopian tubes and not the uterus. If you're sure this is your uterus, then there's no possible way it could be an ectopic.

Was this ectopic or Twins? There’s concern I may be having an ectopic due to one sided pain but my grandmother keeps insisting I am having twins.
