Patriarchal Blessing

How do you go about getting your patriarchal blessing? Is it okay to ask for it, or do you have to be offered it?

I'm 21, married, and I have a 3 year old with my second on the way....my husband and I were married in the church, though not in the temple (he converted while we were engaged and we didn't want to move the wedding date) but are planning on being sealed next year, after this baby comes. I've been thinking about it for a long time, and I would really like to have my patriarchal blessing before we are sealed.

I've been a member my entire life, although I was inactive for a couple of years as a teenager, during my parents' divorce. I don't know if any of this matters, but it's never been brought up so I'm just wondering!
