short term disability

So in about a month I will be having my baby. I started my job when I was around 3 months pregnant and its been going well except for the fact I am way to busy with life! I am also in graduate school as well as interning part time so it has felt like to much for the last couple months. I don't know how I am going to feel about working after baby is born with my 2 other kids and finishing my degree. However I am not sure I want to quit either, I likely will want to cut back if I come back but that's still a If. originally I was told I don't qualify for short term disability which was okay with me- but today I was emailed saying the policy changed and I need to do paperwork with HR for it...what if I get it and then decide I don't want to come back? I would feel so bad...but I just don't know. and then would I need to pay that back? anyone go through this before....
