Help!!!!

I have been feeling awful for the past week, I'm almost 20 weeks but feel like something's wrong. My vision has been blurry and I keep seeing little spots in my vision also. My heart keeps racing every so often and I have headaches I cannot get rid of! My stomach has also been hurting. My midwife is off duty and I don't know whether to call the hospital or go to the doctors or go to the er.
