I’m 16 and I’ve only had sex once and I’ve masturbated many times before. I’ve been fingered, eaten out, blah blah blah, but I don’t think I’ve ever had an orgasm and I can never tell if I came or not... is there something wrong with me?

If you’re not that experienced it may be hard to tell when you’ve orgasmed. It’s kind of tricky to explain but you’ll definitely know when you have an orgasm. 
I would recommend masturbating often. This will give you a better understanding of your body and what turns you on.

Lol for the person who said you are too young, you are not. It is not weird you haven't orgasmed yet. A lot of women have struggles with orgasming (some women haven't had an orgasm until their 40s or what ever). Just try to relax. You know when you have orgasmed.
