Can condoms fail without breaking?

So I know this is probably a stupid question but my S/O is starting to stress that something could have gone wrong last night when we had sex which is also making me stress. We used a condom and checked to see if there was any leakage afterwards and there wasn’t. Though there wasn’t a lot of lubrication on the condoms so is it possible there was a small tear and we wouldn’t have noticed? Or is it possible it failed without breaking?
