Underage contraception

I’m 16, turning 17 next week. Tomorrow I’m going to the doctor with my mom to ask for the birth control pill(she won’t actually be in the appointment with me.) My mom doesn’t know I’m sexually active - she just wants to put me on the Pill ‘in case’ as I’ve been in a relationship for almost two years.

I AM sexually active but because I’m not of legal age to consent - if the doctor asks whether I’m sexually active or not, will she have to tell my mom? And will she have to tell the police(obviously very extreme I guess but) seeing as I’m underage?
