Ripped vag

Ok so lately vagina has been ripping. Not inside it but the skin around it. I have one right above my clitoral hood and then one on both sides where the crease is between the inner and outer lips. I don’t why I keep getting them. And they will heal but the one near my clit keeps opening in the exact same spot again. And then right now I have a yeast infection and I was trying to wipe of some of the discharge before I put on some cream but when I wiped just applying a little pressure, like a normal amount, it ripped. I literally felt it rip my skin. Does anyone know why it’s ripping so easily? I promise I am not that aggressive of a wiper😂
