please help baby butt issues

so for the past couple weeks my baby is having a super hard time to poop I can hear it trying to come out but it doesn't I try a q tip but that doesn't help until it like works It's way out. He's 6 weeks old I keep telling the doctor something isn't right the inside of his butt is red and swollen when he pushes out:( do I just let him cry until he gets it out or what idk what to do
