Milk blister/ bleb- help!!

My son is 7 weeks old and I am dealing with my THIRD milk blister. Had one lanced by a doc, and this recent one is in the same exact spot. I’m doing the whole salt water soak, rub with a towel, all purpose nipple cream routine and am taking sunflower lecithin. But seriously I can’t keep going like this. I nursed my older son for a year without anything like this and now he’s two and I have an infant and I have to be able to chase him around which is super hard with this extensive nipple care regimen- not to mention how painful it is for me.

Anyone else dealt with this? Any advice? Hope? Or do I give up and go formula???
