Ladies with Aetna...

One of my requirements for going straight to IVF it says:

"Bilateral blocked tubes and ectopic pregnancy while under the care of an Aetna provider"

I have/ had both tubal blockage and an ectopic...but I had BCBS. Does this mean I won't get approved? Does it mean like my actual doctor should have been an Aetna provider? Or should have I myself had Aetna?
