Boyfriend moving in and adding him to the lease.

Me and my boyfriend have just decided to have him move in and agreed that we are in a good place to take the relationship to the next step. I went to the park manager of the mobile home community and told her I'd like to add him. She gave me an application for lot lease. Since I have a lease already in effect is this the right application? I have done a little research and have seen normally a tenant amendment or tenant addendum is done if a tenant is being added. If this has happened to anyone, what was the form/forms you had you fill out? Any input would be greatly appreciated.
