Severe cramps

Every month for the past 1.5 yr, I get pretty severe cramps starting the 2 weeks before my period. They are all day and all night and I have to constantly take meds to function. Some times they are so bad that I can’t go to work. Has anyone ever experienced this or knows what it could be? Doctors don’t seem to help much

My cycles are pretty irregular ranging anywhere from 36 to 47 days with some random longer cycles that have been 68 days
