1 gestational sac, 2 yolk sacs and 1 fetal pole

Had my first ultrasound on Friday at exactly 6 weeks. The ultrasound showed 1 gestational sac with 2 yolk sacs and 1 fetal pole. The smaller yolk sac had no fetal pole and the nurse practitioner said she doesn’t think it’s another baby BUT it’s a possibility there’s another baby/twin. Has this happened to anyone? Please let me know your thoughts. We are on femara, monitored ultrasounds and trigger shot. We miscarried over the summer. We are super excited and nervous. Please let me know your thoughts!
