What else do I need for a hospital bag ?

This is what I currently have and I'm not sure what else would I need ? 
I know that I'm missing 
* formula milk
*going home outfit and comfy clothes for myself 
*toiletries for myself such as toothbrush,toothpaste )
I'll also be breast feeding ( Well I'll try to ) but just encase I wanna have formula right there
But not sore if I have everything for the baby ?
Update- hospital doesn't provide us with anything unfortunately

You may not need all those baby clothes besides a going home outfit. Mine just wore a diaper and swaddle the entire time. Hospitals may provide body suits onesies too

Does dad need anything assuming he is staying at the hospital. 
Phone charger. 
Maybe chap stick?
Maybe extra pillow or blanket if you are particular about that.

My hospital provides blankets, wipes, diapers, onesies. So I’m basically just taking a going home outfit for her and just the basic necessities for me.

I was also going to ask if you’ve checked with what the hospital provides. Mine provides everything for baby and will send us home with a hat, wipes, and diapers, so all we need for him is an outfit to go home. They also provide pads/mesh underwear for me and will send me home with some too. You pay for the provided stuff whether you use it or not, so might as well take advantage of it!

Have you checked to see if your hospital will provide? The hospital website has a list for me and they provide all diapers, wipes, formula if needed and even toiletries for the mom and much more
