Child support question

For anyone that gets child support can you answer my question. If I just received a paper to fill out with all of my income info.. that says this.. does that mean paternity has already been established? I call for updates all of the time but they never have any. It’s taken two years for it to come this far so I’m hoping I’m near the end. They wouldn’t be worried about creating a support order and collecting my income info unless he’s already been named as the father right?
