Ultrasounds?

I had a scan at 6 weeks for spotting and then I kept my 8 week ultrasound for piece of mind.  I went in at 11 weeks for my “nurse education appointment” and no ultrasound at that one.  I go for what I believe is just a physical/pap in a few days where I will be 13+4.  I read the papers and it says I could pay $75 extra and get an early gender scan at 16 weeks but if I choose not to do that then my I will have an anatomy scan regardless at 20 weeks.  
My question is, don’t places usually do a 12 week scan? (I’m in the US by the way)

My doctor did 7wks then 18 for anatomy.

No not always every office is different, I only had one ultrasound and that was my anatomy scan.

Not always. It’s also an anatomy scan at 12 weeks. So at 20 weeks it’s basically the same thing except baby will be much bigger.

Depends. Most do two u/s; one between 8-12 weeks and then the anatomy scan. Since you had one at 8 weeks you won't have one at 12

Not necessarily. Every practice is different and since you’ve already had two they probably don’t want to do another unless needed until the 20 week scan. Depends on what your insurance covers as well.
