It’ll differ by office. I think I paid $3,000 ish for mine, that was a couple years ago.

We can't answer that for you because everyone's coverage is different. As someone else mentioned,  it depends on the contracted rates between your insurance company and your health care providers,  as well as your plan's deductible, co-insurance and out of pocket maximum limits. Your best bet is calling your insurance provider and asking them,  but they may need the associated procedure codes in order to give you a quote.
