Needing help with filing generic motion. Anyone

I’m trying to file a generic motion against my sons father. I have sole legal custody and he’s trying to win full custody. The court clerk told me to file a generic motion.

The first part says

“ COMES NOW, in the above captioned case to request the court to:

And the for the following reasons. I already know what I’m going to put on the bottom but the top is confusing the clerk wasn’t much help.

If anyone’s dealt with this before please help. Thanks.
