Questions to ask fertility specialist?

My husband and I have been TTC for over 2 years now and we are going to see a fertility specialist this afternoon. Are there any “must ask” questions you can recommend?

Get hubby checked for possible fertility issues straight away, as well as yourself.
Long term plan.. for example I took clomid for a couple of cycles (didn’t work on me), then FSH injections (worked), but had a plan for next steps if it didn’t (IUI, tubal flushing etc.) I was kept informed throughout the whole process. 
Do they check/scan for follicle growth mid cycle before ovulation (especially if you use a trigger shot)? This is to make sure there’s not multiple ‘ripe’ follicles that could all release eggs (triplets, quads 😱). 
At what point is IVF considered?
