Perineal stitches

With my last baby I had a fourth degree tear, and yet felt like I healed much more quickly (at least the exterior stitches). This time I only had a second degree (am currently 4 and a half weeks pp) and still have a painful and unhealed tear. One side of the tear is hard, lumpy and incredibly tender along the stitches and it still feels like it's tearing apart again anytime I bear down (including blowing nose, e.t.c).

I went in at 3 weeks and was told things looked fine, but haven't healed any further since then. Has anyone else experienced this? I don't want to go in again for no reason and have my 6 week appt in a week and a half anyways.
