it depends what type of work you do. I know for me, I was considered high risk in the beginning, and with lifting a lot I didn't want to risk anything, so I let them know right away. Plus you are going to be very tired (maybe sick) and might not do your best work, at least if you tell them they will be understanding. but it's all in your timing.
