Feeling disgusted

Lately I haven’t been feeling good in my own skin. I don’t want my boyfriend looking at me because I don’t feel good with my body. Any advice? I want to lose the fat and get my shape back. Any advice on what I can do? List of foods I can eat and exercises and how often? I also suffer with depression it’s really hard for me to sometimes motivated to exercise or to eat healthy and it’s frustrating because I have goals.
