8.5 Wks PP - Possible TMI - Is something wrong?

Since I've given birth I've had a hard time with consistent bowel movements. I used stool softeners for 5 weeks (it hurt to poop sometimes to the point of tears) and went off of them hoping I would be regular again.

I took them again 2 days ago and I only just had 2 BMs today. The most recent one was really painful like the beginning of PP, but then I noticed blood in the toilet and around my stool. There was blood when I wiped and when I flushed I saw what looked like a string of blood clots. Has anyone else experienced this? I had a vaginal delivery if it makes a difference, with minor tearing at the top and bottom of my vag.
