Painful left boob

So I have been breastfeeding my son who is 9 months now. Latch on left boob was always uncomfortable. but now it's painful, it's so uncomfortable that I can't let him feed from that side. I have been putting nipple cream, it helps a little, but it starts to hurt again as he feeds.

What could it be?

What can I do?

Should I be worried?
