Okay so I had my first appt for baby #2 today! based off last period nurse said i’m 7 weeks 5 days. then did an ultrasound and predicted i’m actually 5 weeks 5 days. when she was doing the ultrasound she said something else is in there but she couldnt tell yet if it’s another baby or if it’s cyst. any thoughts?
she’s talking about the small dark circle next to the sac.

You could have ovulated later and that would throw your timeline off. 
As far as what the other spot is, it def could be a twin. Or maybe a sub chorionic hematoma- which often resolves itself but needs monitoring. 
But ultrasound techs are allowed to make guesses and give input for a reason 🤷🏼‍♀️

My due date is always 2 weeks off from my LMP estimate because I have 35-38 day cycles. We always go by my ultrasounds so this time, like you, I went in when they thought I’d be 7 weeks and I was 5 weeks 2 days...and no one batted an eye. I also actually have a SCH that was mentioned in the other comment. Mine is more of a squiggle. This was my photo at 8 weeks 4 days...and this particular day I was measuring at 9 weeks 1 day lol
