Transcript/school says I completed practicum when infact I didnt

So I’ve gone to school for veterinary office Administration and I emailed my school to figure out what I have left to do before I got my diploma.

I thought I still had to do my practicum which included a 150 hr placement and they told me I had already completed it. Yes I was enrolled, twice actually(I’ve been in the program for about 4 yrs).

The first time I withdrew because I was unsuccessful in finding a place to accept me.

The second time I did some of the writing at the beginning but then things got rocky and I didn’t find a place again due to poor mental health after losing a baby at 25 weeks pregnant.

So I don’t know if they passed me because they felt bad about my situation or what is going on.

But somehow my transcript says I have a 4.0 and a pass for it ?

Confused and not sure how to go about this. I told them I was pretty sure I still had to do it and they said I already completed it🤷‍♀️
