What to do?

So I am scheduling a gynecologist appointment to get looked at after something that happened to me. If I disclose something can they make you report it or tell people? What happens if I don't want to tell and they ask questions about things they see in my exam or want an explanation? (full std/sti check and unfortunately an exam down there).

I just really don't want to tell them but I feel like their gonna ask questions I don't want to answer. Ik this is probably vague and confusing so sorry if it is hard to understand...
