Help !! Baby won’t stay asleep

My son is 18 months and still doesn’t sleep through the night...I’m also still breastfeeding.

I been monitoring his sleep even more this week and in the past 2 days, both nights he woke up 11 times almost every hour.

I have to give him my boob for him to instantly fall back to sleep.

Since my phone is next to me in bed every time he fussed or woke up I took a screenshot on my phone so I can look at the times. The longest he’ll sleep is 2 hours after I initially put him to bed, after that he’s up almost every hour.
